For All Employer Groups – You may use 1 or 2 to enroll.
- 1. To enroll members individually
- 2. Use Census Management Enrollment spreadsheet for 10 or more employees
- 3. Your Company Information Form (Required)
Five Points Health Benefit Plans also offers to file 1095B & 1094B tax
forms for companies with 50 or more enrolled employees.
